🚽 "Time to head to the bathroom" — imagine getting that notification on your phone, sent by a tiny ultrasound sensor reading your bladder in real time. Many people worldwide struggle with urinary issues, yet for most, the only answer has been diapers. A palm-sized wearable device born in Japan is changing that — and it just won a major award for its impact on children with disabilities.

What Is DFree? The World's First Excretion Prediction Wearable

DFree (short for "Diaper Free") is an IoT wearable device developed by DFree Inc. (formerly Triple W Japan), a Tokyo-based startup. The device uses ultrasound sensors — the same safe technology used in medical ultrasound examinations — to monitor bladder fullness in real time and notify users via a smartphone app when it's time to use the toilet.

The device itself is remarkably compact: just 51mm wide, 36mm deep, and 12mm tall, weighing only 20 grams (less than a AA battery). It attaches to the lower abdomen just above the pubic bone and houses four ultrasound sensors that detect the bladder's expansion as urine accumulates. The fullness level is displayed on a 10-point scale, and when it crosses a user-defined threshold, the app sends an alert.

The origin story is as memorable as the product. Founder Atsushi Nakanishi came up with the idea while studying at UC Berkeley in California after a mortifying experience: he couldn't find a public restroom in time and had an accident. That moment of lost dignity became the catalyst for a company with a mission statement that reads simply: "Move the world one step forward."

Grand Prize at BabyTech Awards 2025-26

On February 12, 2026, DFree was awarded the Grand Prize in the Developmental Support Tools category at the BabyTech Awards 2025-26. The BabyTech Awards is one of Japan's largest recognition programs for ICT and IoT products that support pregnancy, childbirth, and childcare. Now in its seventh year, the 2025-26 edition received 60 entries, certified 42 products through an initial screening, and selected grand prize winners in 13 categories after a rigorous secondary review.

The judges specifically recognized DFree's contribution to easing the burden of toilet training for children with disabilities and supporting their independence and quality of life (QOL). For families with children who have intellectual or physical disabilities, toilet training can be an exhausting, multi-year challenge. DFree makes the invisible visible — showing caregivers exactly when a child's bladder is full and when to guide them to the toilet.

A Decade of International Recognition

DFree's latest award builds on a growing track record of global acknowledgment. At CES 2019 in Las Vegas — one of the world's premier consumer electronics exhibitions — DFree earned three awards including a CES Innovation Award.

The company launched its service for nursing care facilities in Japan in 2017, expanded to individual consumers and markets in the United States and Europe (with a base in France) in 2018, and added services for medical institutions in 2019. To date, DFree has been used in approximately 300 care and medical facilities, serving an estimated 10,000 users. Inquiries have poured in from over 50 countries, signaling global demand.

A significant policy milestone came in April 2022, when Japan's Ministry of Health, Labour and Welfare added "excretion prediction support devices" as a new category under the national long-term care insurance system — making DFree eligible for government subsidy. Several local municipalities have since also added the device to their daily living aid programs.

Japan's Aging Society and the Excretion Care Crisis

Japan offers a window into the future for countries grappling with aging populations. According to a Cabinet Office survey, the most challenging aspect of elderly caregiving is excretion care — cited by 62.5% of respondents.

The adult diaper market tells its own story. In 2013, adult diaper sales in Japan surpassed those of baby diapers for the first time, according to industry leader Unicharm. The annual diaper cost per nursing home resident is estimated at approximately 72,000 yen (about $480), and some facilities using DFree have reported reductions in diaper usage.

But DFree's real impact goes beyond economics. Care sites have reported fewer incontinence episodes among users — but the deeper value is something numbers alone can't capture: dignity. There is a traditional Japanese folk belief that hanging a hydrangea upside down near the toilet wards off the need for help with one's bodily functions. It speaks to a deep cultural value: the wish to maintain one's independence in this most private of activities until the very end.

Transforming Lives of Children with Disabilities

The BabyTech Award specifically highlighted DFree's impact on children with developmental or physical disabilities. Many children with intellectual disabilities struggle to recognize the sensation of a full bladder, and those with physical disabilities may have difficulty getting to the toilet independently.

One parent of a fourth-grader with Down syndrome and severe intellectual disability shared her experience after five years of toilet training: with DFree, she could simply wait for the notification and guide her son to the toilet — zero accidents. A woman with a spinal cord injury who performs self-catheterization reported that being able to visually monitor her bladder volume dramatically reduced her psychological stress.

To celebrate the award, DFree Inc. announced a free trial program for up to 30 facilities nationwide — targeting child development support centers, after-school day services, and special needs schools. Individual users can also access an extended two-week free trial (normally one week) using a special promotional code through March 2026.

Technology That Protects Human Dignity

The challenge of excretion care is universal. Large numbers of people worldwide live with some form of urinary issue, and that number will only grow as populations age globally. What DFree demonstrates is that technology can support one of our most basic and most private biological functions — helping people maintain dignity regardless of age, disability, or physical condition.

The company's philosophy challenges a deeply entrenched default: that diapers are the only answer. By providing an alternative — a tiny sensor that turns invisible biological data into actionable information — DFree is expanding what's possible for millions of people worldwide.
